			<content:encoded><![CDATA[<blockquote class="blogcontent restore">I will go on tour to the <a href="http://en.wikipedia.org/wiki/Arabian_Peninsula" target="_blank">Arabian Peninsula</a>, and lecture about PDA diagnosis and treatment, 25-29/5. My ambition is to share news and views on ductal problems, from my dual  perspective as neonatologist and clinical epidemiologist.<br />
<br />
It will be my first visit to the region, and I am looking forward to it very much, not only for my wish to go on a guided tour of the Desert :).<br />
<br />
First of all, I hope to meet many of you, as I know that many neonatal staff in the Arabic countries are active members here. And, secondly, I will be able to meet the &quot;99nicu co-driver&quot; Habib, aka JACK, for the first time IRL. I am more than grateful to him for joining in 2007 and helping me to &quot;resuscitate&quot; 99nicu.org in 2008.<br />
<br />
I will give lectures in Dubai, Saudi Arabia and Kuwait, and I will post details on times and venues as soon as I get them.<br />

I needed to upgrade the software that 99nicu.org is based upon (vBulletin), and also need to compile new versions of the iOS and Android app.<br />
<br />
Since we started, I have done software installations and upgrades (auto-didactic with computers) but the more complex these softwares become, the more I sweat before I do this technical maintenance. And, I also feel kind of responsible for the hundreds of people browsing 99nicu every day.<br />
If things go wrong, I'd be like a man in a dark forest with the torch just stopped working. I.e. completely lost... and offline.<br />

Keep fingers crossed that we manage to get funding in the future to out-source the whole technical backend of 99nicu.</blockquote>

I am very happy that we managed to launch the event. And, as you may know already, there will be a 2nd EBNEO conference, 13-16 March 2013, in Egypt.<br />

But today I'd just like to tell everyone that review articles of topics presented at the 1st EBNEO in Stockholm are now available on-line, published in a digital supplement in Acta Paediatrica.<br />

What we do? Nothing much really, just enjoying this super-urban city. NY is friendly and tolerant, but also stressful and a spectrum of extremes. Not an unexpected observation. After all, this is a city with a population approx the same as the entire Sweden.<br />
<br />
Sweden just seems - small-scale, uniform and predictable (and boring!). We are used to much less surprises around the next corner. But also less societal tension within a more narrow spectrum.<br />
<br />
We are going back today, but I hope there will be a new trip to the US soon.<br />

We have not yet received the results from the course evaluation, but I believe the conference was a great success! At least, many delegates and speakers told us that it was!<br />

I hope we will not disappoint the many delegates who asked for the 2nd EBNEO conference...<font size="1"><br />

I am very happy we managed to make EBNEO happen. Our original plan was to house the conference at a venue in the archipelago. However, in March when we had to make the definite booking, the number of delegates was far from enough to make the event possible. And, I hereby admit... it was close that we cancelled the whole thing... but we decided to move meeting to Karolinska university hospital. &quot;The rest is history&quot; :)<br />
<br />
Personal impressions and reflections from these days:<br />

<br />
<ul><li style="">Textbooks are out, Internet is the new cool :cool:</li><li style="">Diagnostic tests for sepsis - there is no Holy grail. (but a combo of CRP, IL-8, and six other markers might do part of the trick)</li><li style="">Catheter-related infections CAN be prevented.</li><li style="">Hyperbilirubinemia is difficult to pronounce many times in a row, but it is important to have clear instructions how to handle it.</li><li style="">CPAP is good for preterm infants. And surfactant too.</li><li style="">Individual patient data meta-analysis, sounds boring but is a really cool tool!</li><li style="">Hypothermia is safe and prevents brain injuries in some, but 50% of cooled babies still have an adverse outcome.</li><li style="">Better drugs for treatment of neonatal seizures are needed.</li><li style="">Inotropes starting on D are commonly used, but maybe we should use these drugs in alphabetical order instead. (adrenalin...)</li><li style="">PDA treatment: there is no jury out. Yet.</li><li style="">Oxygenation - don't use to much (term infant), AND don't use too little (preterm infants).</li><li style="">Prevent BPD - and you don't need to treat it.</li><li style="">iNO in preterm infants: so much effort into the EUNO study for... nada.</li><li style="">Want to reduce NEC - travel to Japan!</li><li style="">Prevent pain. If this is not possible, treat it well!</li><li style="">ROP? <a href="http://www.winrop.com" target="_blank">www.winrop.com</a></li><li style="">Does family-centered care make dads sad?</li><li style="">Questionnaire on priorities - it is important to know what the parents are thinking.</li><li style="">Neonatal research idea? Think big! Think together!</li></ul></blockquote>

When we started 99nicu in May 2006, I did not think much about the long-term perspective. I just came up with &quot;a brilliant idea&quot; and convinced a few colleagues that we should start something good and useful. For the world. Ever since I was a medical student, working with international student exchange, I had believed in collaboration outside-the-local-box. When we meet, we get friends. And, friends help each other.<br />
<br />
During these five years, I have seen 99nicu develop into... something more than just a good and useful web site! We created a highly niched community, with a friendly athmosphere. <br />
<br />
Many people have asked for advice, but even more people have shared from their expertise. Geographical boundaries or time zones have not been limiting factors.<br />
<br />
For myself, 99nicu has changed my life. I have met new friends. I have learned about how neonatal medicine is practised around the world. And... I have been reading emails... :)<br />
<br />
Former and current 99nicu Team fellows, myself included, have spent many hours house-keeping the forums. But, as a community is made up of its members <font size="1">(I realize this sounds like some kind of communism, but it's true)</font>, 99nicu has thrived only thanks to you. Therefore, I want to salute you.</blockquote>

Researchers  at the Charité university hospital in Berlin, Germany, had (<font size="1">convinced?</font>)  a pregnant woman to deliver her baby in an MRI camera!<br />
<br />
Spectacular experiment! Creating buzz and <a href="http://www.dailymail.co.uk/health/article-1336521/Doctors-Berlin-hospital-produce-MRI-scan-baby-moment-birth.html" target="_blank">headlines</a>!<br />
<br />
If  you ask me, this is a great combination of great PR work and serious  research. Everyone involved gets fame and cred (<font size="1">also the baby I hope</font>),  and the research group will be granted to take their work to new  heights. We need to learn more about the birthday, probably the most  hazardous day in our lifes (<font size="1">if we don’t consider our very last day,  lethal as it is...</font>) The actual journey to life, so often complicated (<font size="1">as  you know</font>), needs more researched. <br />

Lilla barnet fond).</a> The idea was to create a nation-wide charity foundation for research about newborn babies and their future health. Similar to March of Dimes. The &quot;Lilla barnets fond&quot; was officially founded late 2008, and since then we have worked hard (as volunteers) with strategic issues, including fund-raising  and PR.<br />

This is a follow-up report with royal gossip! :)<br />

Yesterday we achieved a major milestone. <a href="http://en.wikipedia.org/wiki/Prince_Carl_Philip,_Duke_of_V%C3%A4rmland" target="_blank">The Swedish Prince Carl Philip</a>, the official patron/protector of Lilla barnets fond, handed over the first research grants to the research projects, which were top-ranked by the foundation's scientific committee. In total, the researchers recieved 260.000 SEK (about 27500 EUR / 38000 USD).<br />

After the meeting, I and a few other board members met up with Prince Carl Philip and informed about our future ideas. Prince Carl Philip was very interested of neonatal care issues! He even wanted to be specifically involved in a future campaign...<br />

I am the single paediatrician and neonatologist of our institution. Junior consultants stay with us for very short time till they get the feel of private practice.They often start their own clinics or small hospital. This results in chronic lack of second in command. Though we have medical officers we need to take the lead.<br />
So a typical starts at 7 am and ends at 10 pm with 30 minutes break.<br />
We need to work 6 days a week and be always on call. Now we are in the middle of dengue , chickengunya and viral fever season. Add to the is the NICU PICU and OP services. WE have our hands full.Trying to catch up or do some good study needs enormous will power. <br />
<br />
Lucky  i have an understanding wife and a medical student daughter and middle school studying  son. They are surprised when i reach home by 9 pm and always ask me whether i need to go back. I compensate for all these by staying at home on sundays . But we have some pressing work to do even on sundays . I always wonder how all others manage to do !<br />
<br />
When i get bogged down by the work i swear that i will stop my practice at the age of 60 yrs and enjoy retired  life.Hope this comes true 10 yrs from now.</blockquote>

Are we justified in doing heroics to save the baby or just  decide for the parents whether to continue the critical care for the newborn? If i make a decision for the parents can i call myself as a health care professional or some one with a ego to decide to let the  baby  live ?<br />
If the parents are poor and if the critical care is going to cost much ,which will increase the debt burden is it better for them to have the next baby which is economical ( i am sorry for the word). These questions are always with me whenever we get a poor father.<br />
I can not tell whether i have made all good decisions but few times i got verbally abused when after all our efforts the baby dies or they want to go home because of financial reasons<br />

Tempted by the &quot;short notice bonus&quot; for the night shift (money can drive me to do things too...), I volunteered, went to the NICU, changed clothes and was handed over the new pager.<br />

The new pager - a massively bulky something! (new and modern technical things tends to get smaller and smaller. Maybe this was the exception from the rule).<br />
<br />
A calm evening. Went to bed at midnight. The pager went off at 3 am.<br />
<br />
Woke up, fiddled with the pager, looking at the large NOT-backlit LCD-screen in the dark. The telephone number was shown in the upper left corner with very <font size="1">tiny-tiny</font> digits. Damn large screen, why the **** are the digits displayed so small in the upper left corner ??!!<br />
